How to use it:
1. Fetch data.json once. It is a keyless, unpaginated static dataset.
2. For streaming ingestion, read action-records.ndjson or institution-records.ndjson one complete record per line. Validate each line against its matching items schema in schema.json.
3. Search action_records by institution, agency, action.action_ids, and the typed search_terms array.
4. Search institution_records by exact CMS Certification Number, institution_name, state, source_classification, provider type, and collection_id. Only the 23 RNHCI rows carry provider_program_evidence. The CCN is the primary key; names are not deduplicated.
5. Query sector_aggregates.measurements by metric_id. Respect universe_id, unit, value_status, status_reason, and relation before doing arithmetic.
6. Use sources.json to reverse-map each deduplicated reviewed URL to its record IDs, collection IDs, action IDs, agencies, labels, and publishers.
7. Filter with facets or the code fields in institution_type, identity, action, and institution_record_facets.
8. Read outcome and does_not_establish together. Never quote one without the other.
9. Treat coverage_gaps as first-class results. Null, blocked, withheld, missing, and excluded are not zero.
10. Follow official_sources for every publication claim.
11. Verify publication.json, receipt-bindings.json, and evidence-manifest.json against their adjacent SHA-256 values in manifest.json before relying on provenance. The release manifest is deliberately not self-hashed.

Non-negotiable interpretation rules:
- A settlement is not an adjudicated adverse finding unless the exact record says otherwise.
- Allegations remain allegations.
- Do not infer payment, collection, present status, current compliance, motive, concealment, corruption, or prevalence.
- Do not merge source universes or years.
- source_observed means only that a receipt-bound aggregate was reproduced for its exact universe; it does not override source scope or turn an administrative code into a finding.
- CMS provider-program codes 01 and 04 are voluntary. Certification-survey A/B codes are not present-day compliance findings. Blank ownership-change dates are not evidence of concealment.
- The dataset excludes donors, natural people, contacts, addresses, and unreviewed identity guesses.
- This is not a census, faith ranking, corruption score, or bad-actor list.
